It does not have to be good poetry.  You do not have to fully grasp any of the poetic terms or conventions like rhyme, meter or free verse.  You do not even have to have ever read a truly good poem in your life to write a heartbroken poem that can help you feel better and perhaps help you recover from the pain of a break up.

An essential beginning step in getting over a break up or any depressing circumstance is merely confronting the pain.  Although it may feel better to steer clear of dealing with the actuality of the circumstance, you cannot truly move past it without dealing with it and experiencing the pain, at the very least for a while.  So dealing with whatever painful circumstance is occurring is important.  A heartbroken poem can help you cope with those painful emotions when you have split with a person you love.

Do not be anxious about whether or not it is good or not. No one else ever has to read your heartbroken poem.  It is for you and you only.  Simply write down your thoughts, as difficult as that may be.  You can write in simple language.  Do not attempt to mimic the poets of the 17th and 18th century.  Write like you speak, and separate the lines where it seems normal to you.  In reality, you can start by writing one big paragraph loaded with all your emotions and anything you want to convey, just to get it out.  Then you can go back and organize your ideas and feelings into a poem.

Once you have written a heartbroken poem, you may want to write more about various areas of the pain.  That is good.  Write down everything you can, and that will assist you to face the pain. Writing the poetry will most likely be a very emotionally charged time for you.  Do not attempt to stop it.  Simply let the pain out and you will be better able to move on.
